Autoimmune hypophysitis: a study of natural course

چکیده
Autoimmune hypophysitis is a rare autoimmune endocrinopathy. It is possible that the prevalence of autoimmune hypophysitis is underestimated. 1,2 The reported incidence of autoimmune hypophysitis is one in nine million. 3 A recent study reported an estimated incidence of autoimmune hypophysitis in India as 2.4 per ten million population. 4 Increasing recognition of the disease is contributing for the recent increase in the incidence. In addition to the spontaneously occurring autoimmune hypophysitis cases, a large number of iatrogenic cases have been reported with the use of CTLA4 antibody, ipilimumab. Hypophysitis has been reported in up to 17% melanoma cases treated with ipilimumab. 5 With the increasing use of ipilimumab in the treatment of melanoma, addition of these iatrogenic autoimmune hypophysitis cases may further increase the incidence of autoimmune hypophysistis.

Ovulation induction and normal pregnancy after panhypopituitarism due to lymphocytic hypophysitis.
BACKGROUND Lymphocytic hypophysitis is an unusual autoimmune disease that causes partial or total hypopituitarism and often is associated with pregnancy. Only four spontaneous pregnancies have been reported after this disease. We report a case of ovulation induction in a woman with this antecedent as well as the course of the subsequent pregnancy. CASE Ovulation was induced with gonadotropins...
متن کاملCytotoxic T-lymphocyte-associated antigen-4 blockage can induce autoimmune hypophysitis in patients with metastatic melanoma and renal cancer.
Cytotoxic T-lymphocyte-associated antigen-4 (CTLA-4) is an immunoregulatory molecule expressed by activated T cells and resting CD4CD25 T cells. In patients with advanced melanoma, our group reported that administration of anti-CTLA-4 antibody mediated objective cancer regression in 13% of patients. This study also established that the blockade of CTLA-4 was associated with grade III/IV autoimm...
متن کاملLymphocytic and granulomatous hypophysitis: experience with nine cases.
OBJECTIVE Lymphocytic hypophysitis and granulomatous hypophysitis are rarely encountered. The aim of this study was to demonstrate their clinical peculiarities among pituitary disorders and to provide an approach for their clinical management. METHODS In a retrospective study, we reviewed our surgical experience with nine patients harboring hypophysitis. The series included six cases of lymph...

A 56-year-old female presented with idiopathic hypertrophic cranial pachymeningitis manifesting as headache, hypopituitarism, and diabetes insipidus, mimicking lymphocytic hypophysitis. Five months later, she complained of double vision and unusual right facial sensation. The diagnosis was based on magnetic resonance imaging, angiography, and meningeal biopsy via transsphenoidal surgery, and ex...

A 70-year old man with a 14 year history of Sjögren syndrome, interstitial pneumonia, and autoimmune hepatitis (AIH) was admitted to our hospital due to hyponatremia with a one month history of fatigue, thirst, and nausea. Laboratory tests on admission revealed that this patient had a central adrenal insufficiency. Pituitary magnetic resonance imaging (MRI) further showed swelling of the stalk ...
